In surgeries where a trochanteric fixation nail has been inserted—typically to stabilize proximal femur fractures—the nail sometimes needs to be removed either due to non‑union, incorrect positioning, or for later procedures. The 357.421 guide plays a critical role by helping the surgeon properly align the extraction screw to the proximal end of the nail. This precise alignment ensures that the extraction force is applied centrally and evenly, reducing the risk of damaging the remaining bone or the implant during removal. Constructed with robust surgical‑grade materials, the Extraction Screw Guide has sufficient rigidity and dimensional accuracy to maintain alignment despite the mechanical loads associated with nail extraction. Its long reach (~ 14.75") allows access in deeper anatomical regions, considering soft tissues and surrounding structures, while still allowing room for the required extraction screw and associated tools. The guide’s design helps minimize off‑axis forces that could otherwise cause binding or damage either to the nail or to the surrounding cortical bone. In clinical workflow, the 357.421 is used after disengaging the locking mechanism of the trochanteric nail (via the extraction screw and hexagonal driver, etc.).
